2024 - 2025 Private Lesson Request Form

Parents:   please fill out the private lesson time form (pdf link below) or email me with the information needed even if you have already notified me of your preferences.  I want to be sure I don't forget anyone's requests!  Length of lessons offered:

1. Elementary students’ lessons are for 30 minutes.
2. Middle School students’ lessons are 45 minutes in length.
3. High School students’ lessons may be 45  or 60 minutes in length.
4. Times listed below may be adjusted 15 minutes earlier or later depending on where the elementary lessons fall.  Please take into account there are 4 students moving up to Middle School this year with all scheduling problems that presents.

DUE DATE FOR LESSON TIMES REQUEST:  JULY 31
